After troubleshooting this issue, our internal testing shows that the issue is a result of a bug in iOS 9.3.1 and that the issue does not occur in devices running iOS 9.3.2 beta or earlier versions of iOS. The FW update tool on the sandisk site is for the iXpand USB 2.0 product. It has a picture of the product in the tool. This is not for the iXpand USB 3.0 product.
